Request A Duplicate Diploma or Transcript

If your diploma has been lost, damaged, or destroyed, you may order a replacement. There are no duplicate copies held on file. Processing may take as long as 10 business days. Please note that diplomas are issued in the format currently used and may bear the signatures of the present High School and State officials, not necessarily those who were in office at the time of your graduation.

Reissued transcripts can be mailed to the address(es) indicated on the form or received at time of request.

It is MANDATORY that you sign this form before returning it or we cannot process your request.

Money Orders, Cashier Checks, Company Checks and Personal Checks made payable to Livingston Parish Public Schools can be accepted. Cash is accepted ONLY in person (must have correct change).
